I think I’ve seen this or similar before. It’s terrifying. Also it feels like someone has built an illegal extension out into the neighbouring alley and slapped a bog on it.
There are soooo many questions raised by this.
It’s probably in a basement and the sewage line is a few feet above ther floor.
That is… worrying.
Why? It’s not uncommon for a sewage line to be above the floor level of the basement.
It was probably an unfinished basement that someone added a toilet to. You can’t put the toilet below the sewage line.

You can definitely cut off the circulation to your legs unless you go side-saddle.
And it’s convenient to kneel in after a bender. Sucks for lying on the floor between heaves tho.
